UPDATE: Apparently this last one did get mentioned - see the comments - although maybe it was just mixed in among all the music references that went over my head? Because I don’t recall.
At any rate, I just Googled the phrase “my bad” to see if I could say that it originated in the 90’s. The answer is highly unclear, but I enjoyed this sentence in the Wikipedia entry for “mea culpa”:
In today’s American vernacular, people often say “my bad.”
